You must enter the date using the 4-digit format: mm/dd/yyyy. To enter a date of birth, use the mouse to click on the appropriate field. For data fields, use the mouse to click to the appropriate field and enter information.

If the dates are entered correctly and the error continues, check your regional date settings in the control panel:

• Click Start, Settings, Control Panel, Regional Settings, Date.
• At the Regional Settings screen, click the Date tab.
• Click the arrow at Short date style and select MM/dd/yyyy.
• Click Apply, OK.

CMS-SA does not accept February dates greater than the 28th. For leap year birth dates (02/29/yyyy), enter the date as February 28.
