Receiving message: Value greater than 60, when entering Reading Comprehension raw score

I am receiving a message on my scoring assistant that states: "Value greater than 60" when entering my raw score for the Reading Comprehension item set 9-12. Why am I getting this?

Resolution:

Please check your Record Form and make sure it says "UPDATED" on the top right hand corner. If it does not say "UPDATED", you are using an older record form which does not match the scoring assistant values. To order the record form, please contact Customer Service at 800-211-8378.
