Answer:A separate Record Form is available for each Scale in order to provide flexibility for customer needs. For instance, these record forms can provide more efficiency when conducting research, and for multidisciplinary and arena assessment teams. The standard Record Form (combining all 3 Scales) contains all the information and items found in the individual Record Forms. |
