Answer:On the initial printing of the Record Form, a column was omitted from the right side of the grid on Item 8. There are no correct answers in this column. All correct responses on this item are in the two left-hand columns. Add a column to the right side if you need to record an incorrect response. This was corrected for the second printing of the Record Form.
